Lambeth's integrated care model has been given the go-ahead, aiming to provide more joined-up and proactive support for residents. The Lambeth Together Care Partnership Board ratified the Lambeth Integrator Model at its meeting on 3 July 2025, paving the way for enhanced neighbourhood working across the borough. The goal is to have integrated neighbourhood teams up and running by April 2026, supporting groups with frailty, multiple long-term conditions, children and young people, and mental health needs.
The Lambeth Integrator Model is a partnership between Guy's and St Thomas' NHS Foundation Trust (GSTT) and the Lambeth General Practice Provider Alliance (LGPPA). The board ratified the proposed model and the use of £250,000 for integrator development. This funding, provided by SEL ICB, will be held by GSTT on behalf of the integrator and used to respond to needs identified through a maturity matrix exercise. Principles for spending will be ratified by partners overseeing Lambeth Together priorities, as detailed in Appendix 1 - Lambeth Integrator Response 25 6 25.
Final approval for the integrator memorandum of understanding (MOU) has been delegated to the Neighbourhood & Wellbeing Delivery Alliance Leadership Board, with regular updates to the Lambeth Together Care Partnership Board.
The implementation timeline for the Lambeth Integrator Model is structured in three phases:
- Short term (April-June 2025): Developing a high-level delivery plan.
- Medium term (July-Dec 2025): Detailed design and initial delivery of the integrator model.
- Long term (January 2026 onwards): Embedding and iterating the integrator model, with integrated neighbourhood teams expected to be operational by April 2026, as outlined in the Neighbourhoods document.
